Your First Visit
When you first arrive at our dental office, please come to the front desk and ask for a new patient form. Filling out new patient forms only takes a few minutes and let’s us know about any dental concerns, existing medical conditions and accommodations you’d like for our team to keep in mind. Once we call your name, you’ll be escorted to one of our operatories and one of our dentists will meet with you shortly. From there, they’ll perform a brief exam and a cleaning (time permitting) as well as discuss any treatment recommendations based on the current state of your oral health.
New Patient Forms
Filling out new patient forms is one of the easiest ways you can educate us on your specific oral health needs. However, they also let us know about any current medical conditions we should know about, any symptoms or issues you’re experiencing, medications you are currently taking, and other important notes our team should be aware of. Please arrive at the dental office 15 minutes ahead of your scheduled appointment time so you can adequately fill out a new patient form.

Dental Insurance Welcome
We are happy to be an in-network provider with many different dental insurance plans and look forward to helping you and your family maximize benefits to the absolute fullest. Every dental plan comes with their own unique stipulations, which we understand can be confusing at first. That’s why our dedicated team is happy to walk you through them before you start treatment, ensuring that you understand your coverage and how much you’ll need to pay ahead of time.
Financing Available
If you do not currently have dental insurance, either because you don’t find it affordable or you’d prefer other methods of payment, we have another option worth exploring. CareCredit is a third-party company that helps patients cover their dental care costs over multiple months, rather than having to pay for services all at once. Not only do these payment plans come with little to zero interest attached, but they help you pay for services that even dental insurance does not traditionally cover.
